Description:
Key Responsibilites Manage the Digital Clinical Hub: Oversee the processing of clinical correspondence for all aligned Southport and Formby GP practices Utilise EMIS Functionality: Ensure efficient and accurate recording of clinical information using EMIS software Line Management: Provide leadership and management to a team of Digital Clinical Hub Care Coordinators Reporting: Deliver service reports to Southport and Formby PCN, including Key Performance Indicators (KPIs) and other monitoring requirements Team Development: Support the team in accessing relevant training, development, and supervision sessions Resource Management: Assist in the creation and management of EMIS resources for the digital hub Liaison and Communication: Maintain continuous communication and engagement with PCN practices, PCN stakeholders, and wider regional stakeholders. General: Process Improvement: Assist the PCN in developing and refining processes to enhance the coding, filing, and processing of clinical correspondence, benefiting both practices and patients. Line Management: Provide comprehensive line management to the Digital Clinical Hub Care Coordinator Team Leads, including supervision, appraisals, and day-to-day operational tasks. Support Practices: Aid GP practices aligned with the Digital Clinical Hub in coding and filing all relevant clinical correspondence.
Patient Care: Contribute to direct patient care by expediting care, arranging appointments, and enhancing the readability and understandability of electronic health records. Utilise Best Practices: Implement current models of practice such as workflows and templates for accurate recording of clinical information, and assist in developing appropriate reporting systems. Accurate Coding: Ensure the precise coding of records using standardised SNOMED coding to improve data quality. Audit and Analytics: Engage in audit and analytics work to support the digital clinical hub and PCN practices.
Digitise Workflows: Assist member practices in digitising workflows to enhance efficiency. Collaborative Support: Work with colleagues to share skills, expertise, and knowledge to improve processes for all GP practices within the PCN. Administrative Support: Provide administrative assistance to GP practices requesting additional help with clinical correspondence or administrative tasks, as agreed with the PCN. Training and Development: Attend relevant training sessions to support the development of the Digital Clinical Hub and your personal growth.Service Reporting: Deliver service reports to the PCN on team performance, KPI measures, and contribute to relevant subgroups within Southport and Formby PCN and Southport and Formby Health Ltd.
Relationship Building: Develop and nurture relationships with team colleagues, wider PCN colleagues, and practices across the PCN Stakeholder Engagement: Liaise with PCN stakeholders and wider regional stakeholders to foster continuous communication and engagement with the service. Innovation and Improvement: Contribute to the ongoing improvement and innovation of the service, identifying opportunities for enhancement and implementing changes as needed. Quality Assurance: Monitor and ensure adherence to protocols and standards to maintain high-quality service delivery. Operational Efficiency: Manage resources effectively to ensure the efficient operation of the Digital Clinical Hub.
