REQUIREMENTS
Licensure / Certification / Registration:
- Required Credential(s):
- One or more of the following:
- Case Manager credentialed from the Commission for Case Manager Certification (CCMC) obtained prior to hire date or job transfer date.
- Counselor obtained prior to hire date or job transfer date.
- Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date.
- Social Worker credentialed from the Texas State Board of Social Worker Examiners.
- Respiratory Care Practitioner obtained prior to hire date or job transfer date.
- Respiratory Therapist credentialed from the National Board for Respiratory Care (NBRC) obtainedprior to hire date or job transfer date.
- Social Worker obtained prior to hire date or job transfer date.
Education:
- High school diploma/GED with 2 years of experience, or Associate’s degree, or Bachelor’s degree required. Master’s degree preferred.
Work Experience:
- 5 years of experience required.
- 10 years of experience preferred.
- 2 years of leadership or management experience required.
- 5 years of leadership or management experience preferred.
Responsibilities:
Responsibilities:
- Formulate and implement operational strategies and initiatives in the case management program.
- Direct and evaluate all services provided, including care plan development, service level determination, and complaint management to achieve performance and quality control objectives.
- Provide specialized expertise, ensure resource alignment with system-wide initiatives, and update appropriate parties on case management strategies, initiatives, outcomes and issues
- Successfully partner with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinicalvariation in care.
