Description:
KNOWLEDGE/ EDUCATION REQUIREMENTS:
A. Bachelor’s degree required, Master’s degree preferred
B. Graduate of an accredited RN nursing program, with a current New York State license in Registered Nursing
C. Strong Project Management/lead experience
D. Completes additional training for care management, SBIRT, Depression and Anxiety screening to support assessments for mental health and substance use disorder patients as applicable
E. Demonstrate a willingness to accept direction from providers and Sr. VP/ CNQO
F. Be keenly aware of the importance of confidentiality in all aspects of the position Must be able to work well with a team, facilitate team activities, and professionally represent the organization with internal and external partie
Responsibilities:
- Oversee all current quality initiatives, while continuously assessing new opportunities to improve health outcomes.
- Provide leadership to ensure the provision of patient centered care management with efficient and cost effective operations.
- Oversees the coordination of efforts for care coordination with RN care managers and community health workers and monitors outcomes for both roles.
- Educates providers on the different care management programs available and their requirements for participation.
- Supports staff education in orientation and site meetings to ensure documentation is completed to fulfill quality measures
- Lead Care Management efforts and evaluates overall performance of the care management programs by gathering, analyzing and interpreting data and metrics.
- Coordinates the Prescription team meetings and reviews prescription workload, dividing evenly between team members. Monitors compliance with prescription processes for corporate compliance. Manages 340 B prescriptions for opportunities to
- Leads special projects and supports clinical projects, updating project work plans, policies and procedures and reports. Ensures reports are completed timely and adheres to regulatory standards and organizational policies and procedures.
- Participates in quality improvement, in-service workshops, continuing education and peer review as requested by management and supervision
- Engage in Patient Centered Medical Home initiatives, including participating as part of the Clinical Care Team
- Assist with and promote all processes associated with the Coleman initiative
- Any other duties and responsibilities as requested by management and supervision
