Licensure / Certification / Registration:
- Project Mgmt Professional Licensure required relevant to state in which work is performed. preferred
Education:
- High School diploma equivalency with 5 years of applicable cumulative job specific experiencerequired, with 2 of those years being in leadership/management OR Associate’s degree/Bachelor’sdegree with 3 years of applicable cumulative job specific experience required, with 2 of those yearsbeing in leadership/management.
Responsibilities:
This role will have oversight of the Ascension National Post Acute Network strategy, engaging with market leaders to establish and manage partnered networks of Post Acute providers. The Director will collaborate with national leaders in Acute and Ambulatory Case Management, as well as market leaders, to enhance discharge referrals, achieve Continuity of Care goals, and improve quality of care outcomes. This position will lead the national PAC Network strategy, defining expectations for markets, establishing criteria for network size, performance oversight, and cadence of relationship touchpoints (PAC provider visits, Performance discussions, Town Halls).
- Lead biannual network reviews and maintain the PAC provider master list, ensuring alignment with performance and quality goals.
- Serve as the primary contact for national Continuity of Care (CoC) scorecards, supporting accurate reporting and data analysis.
- Develop and implement PAC Network standards, including provider performance criteria, network size, scope, and scorecarding.
- Oversee the PAC contracting/commitment process and ensure integration of Continuity of Care goals across the network.
- Manage PAC Network Manager roles and relationship processes, including performance meetings, onsite reviews, and town halls.
- Provide ongoing support for the Post-Acute Network Database (PAND) and ensure efficient network management through continuous process improvement.
