Description:

Reporting to the Accountable Officer at Place in Bradford District and Craven, the Director of System Transformation will lead the development of our vision for fully integrated pathways of care for all population and care groups. They will ensure this reflects the aspirations and plans of all relevant partners and aligns to the strategies and plans of the West Yorkshire Integrated Care Board (ICB). This role means they will be accountable and responsible for providing overall leadership for Bradford District and Cravens Health and Care Partnerships priority and enabling transformation programmes, working with partners to maximise a coordinated, integrated approach to improving the health of the population we serve. The Director will also be accountable for the development and delivery of the highest quality primary and community services across BDC Place, securing better health and care outcomes for our population and placing a strong emphasis on promoting independence and ensuring that the interests of patients and the community remain at the heart of discussions and decisions.
Working with the Associate Director of Primary Care in Bradford District and Craven, the Clinical Advisory Board, the West Yorkshire ICS and NHS England you will assure and develop the quality of general practice and Primary Care Network provision for Bradford District and Craven residents, within budgetary constraints. Additionally, they will be responsible for building partnerships and collaborating with wider HCP and system leaders including provider collaboratives, public health, primary care, local government, voluntary and community sector, other partners and local people to make real transformational differences for the population. You will lead partnership working with Local Authorities, West Yorkshire collaboratives, the Local Medical Committee and other partners as well as with local people to make real transformation differences for our population. The Director will develop, hold and articulate a vision for fully integrated pathways of care for all population and care groups, ensuring that integrated teams and projects fully adopt a continuous improvement approach.
They will work closely with colleagues to develop the programme boards as commissioners of care for their populations, using data, insight, health economics, and evidence to identify, prioritise, and drive transformation. As a member of the ICB the Director of System Transformation will be jointly responsible for the planning, allocation and management of resources to meet the four requirements of integrated care systems (ICSs) and in doing so for delivery of the West Yorkshire Health and Care Partnerships (HCP) mission: - To improve outcomes in population health and healthcare and so reducing health inequalities; - To tackle inequalities in outcomes, experience, and access and so managing unwarranted variations in care; - To enhance productivity and value for money and so using our collective resources wisely; and - To help the NHS support broader social and economic development and so securing the economic and social benefits of investing in health and care. The Director of System Transformation will be accountable for developing and delivering our collaborative commissioning approach, supporting the delivery of our strategic objectives. They will help set the tone within the organisation and the wider system, providing outstanding professional, compassionate, inclusive, and corporate leadership across the NHS and the wider health and social care system to ensure the best possible standards of healthcare on behalf of the population.
As a professional and credible leader, the Director of System Transformation will work through professional networks across the ICS and regionally where relevant to provide collective leadership, engaging in peer review and challenge to improve overall practice and ensure shared learning. They will uphold the Nolan Principles of Public Life, the standards for NHS Boards set out by the Professional Standards Authority and the NHS Leadership Framework competencies. They will also participate in a regular on-call commitment
