Description:
Sherwood Forest Hospitals NHS Foundation Trust is seeking a dedicated and proactive discharge specialist practitioner to join our team, focusing on discharge coordination. The successful candidate will play a pivotal role in ensuring the timely and effective discharge of patients across the Trust. Working closely with ward-based multi-disciplinary teams and the discharge hub, you will:
Ensure the provision of specialist advice.
Develop and implement policies to manage the discharge process.
Ensure all patients with long lengths of stay over 14 days have clear discharge plans.
Conduct Reason to Reside checks for all patients.
Participate in the Length of Stay (LOS) report weekly meetings and other relevant meetings.
