Description:
- To act as an autonomous nurse taking on going responsibility for the care coordination of appropriate patients.2. Be responsible for the assessment, planning and implementation and evaluation of nursing care to individuals, groups of patients and their families on the district nursing caseload encompassing a rehabilitative and self-care approach delegating activities as appropriate.3. Promote clinical excellence and evidence based approach to care by complying with the agreed Trust policies and procedures maintaining own clinical competencies within a clinical leadership role.4.
To act as the practice link with GPs-in and Out of Hours and other general practice staff.5. Liaise with other Health Care and Multi-Agency professionals to provide a multi-disciplinary approach to care, accepting referrals for assessment as per referral criteria.6. Demonstrate, teach and supervise the nursing care that can be safely delegated to the patient/carer giving support as necessary.7. Ensure that equipment and aids, necessary for effective care, meet the needs of individual patients and are adequately maintained.
Demonstrate and explain safe use of equipment for patient and/or carer where required.8. To ensure that own and community nursing team implement the End of Life (EOL) Strategy including adherence to all EOL tools, acting as a clinical expert in EOL care provision covering the 24 hour period.9. To ensure a safe working environment advising on all aspects of safety in the home.10. To be responsible for monitoring the provision of health care given to patients by all grades of staff within the District Nursing Team including following up assessment visits undertaken by other members of the team in a timely way
