Description:

We are seeking a dynamic, highly motivated Diversion Case Manager who demonstrates our values of compassion, accountability, responsiveness, and empowerment; to our clients, families, and each other.
The Diversion Case Manager is a staff in the Diversion Program collaborative initiative that emerged from the Street Youth Planning Collaborative and is in partnership with the Catholic Children’s Aid Society, Social Planning and Research Council, Good Shepherd Youth Services, and Wesley (employer).
The Diversion Case Manager works to coordinate care between the partner organizations and cases manage clients in the community. This position will also coordinate care for the Diversion bed youth located at Wesley Youth Housing through direct case management, family reunification and mediation, assistance to obtain safe housing and co-ordination of the support Wesley Youth Housing staff provide to Diversion beds in Diversion Case Manager non-working hours.
This position is a unionized, full-time, permanent position. The rate of pay is $23.21 per hour.
Responsibilities:
- Will successfully engage and support youth to transition to and sustain successful, permanent living arrangements.
- Develop policies and procedures as they pertain to the Diversion Bed program at Wesley Youth Housing, complete with an analysis of processes and revamping when appropriate.
- Manage issues pertaining to case management and Diversion operations that may arise.
- Provide case management for the Diversion Bed Program.
- Complete statistics, client files, case notes, incident reports, and other administrative documentation, as required.
- Support the assessment and intake of youth to Diversion beds applying a risk lens and communicating any challenges or concerns in a timely manner.
- Coordinate additional Mental Health supports when required
- Assist youth in areas such as job searches, self-care, problem-solving, healthy lifestyle choices, budgeting/money management, community, tenancy/landlord issues, and various other instructional and training opportunities.
- Attend internal and external team meetings, and complete required documentation, data collection, and reporting within required timelines.
- Maintain positive working relationships with partner agencies.
- Coordinate with child welfare, shelter staff, and community resources to ensure gaps are met and the best service delivery is available, supporting youth who can be diverted from homelessness.
- Assist youth in maintaining safe, affordable housing options.
