Description:

Facilitate Care Services are a rapidly growing domiciliary care company with the support and passion of a small, exclusive care team.
We are looking for a new member of our team to join us in our exciting expansion and assist us to provide high quality care to more people in their own homes. We are a client focused organisation and wish to offer the opportunity for another dynamic Care Coordinator to join our team, ensuring person centred care is provided to our growing number of satisfied clients.
Maybe you are looking to progress the career ladder and be in charge of your own client base, to manage a care team and implement changes to make the improvements you have identified as needed.
A working knowledge of legislation and requirements are required of the successful candidate, as well as good team working, empathy and flexibility.
Must have NVQ level 3 and be willing to work towards the NVQ 5 Diploma in Leadership-Health and Social Care qualification.
The office working hours expected are Monday to Friday 09.00-17.00, with additional hours of work over the weekend as and when required.
There is an on-call element of the role, to support the care team out of office hours. The successful candidate will also be required to share shift cover during carer sickness and annual leave.

REQUIREMENTS:

Possess a full British driving license
Computer literacy
Excellent communication skills to communicate in verbal and written English
Experience in the care sector
Be flexible in working to ensure all needs of the business are met
This is a full time job role in a 24 hour service, this may include some working outside of office hours

Responsibilities:

Meeting with clients and assessing their care needs and discussing their preferences
Completing Care plans and ensuring they are updated
Manage your own client portfolio
Support care staff through observations, spot checks and supervisions
Arrange relevant training for carers supporting specific client groups
Supporting the scheduler to ensure all care shifts are covered
Providing customer service both in person and on the telephone
