LifeScape is looking for someone who is both personable and has great attention to detail. The medical team is looking for someone who can work in a team, but also works well independently. Searching for someone who enjoys organizational tasks, is technically proficient, and looking to make a difference in the lives of others.
This position is responsible for supporting adults with disabilities, while conducting medical-related services in our nursing environment. This position will be performed in care environments that include residential, hospital, or day-services settings/sites.

PROVIDE CARE, SUPPORT, AND MEDICAL APPOINTMENT ASSISTANCE FOR ADULTS WITH DISABILITIES:

  • Scheduling medical appointments
  • Driving company vehicles, including wheelchair accessible vans.
  • Complete vehicle mileage logs
  • Provide direct care support.
  • May administer medication as needed.
  • Assist with dietary and hygiene needs, positioning, and transferring, according to each individual’s appropriate need for assistance
  • Facilitating communication between office nursing personnel and outside medical professionals
  • Ensure that all paperwork is filed correctly in a timely manner
  • Complete and/or follow-up on the completion of documentation from medical appointments
  • Report and document all applicable care and support information, using appropriate forms
  • Float as assigned from one environment to another
  • Attend, complete, and maintain trainings as required
  • Attend and actively participate in required meetings
  • Assist in emergencies
  • Complete other duties as assignedMEET ALL REPORTING, COMMUNICATION AND DOCUMENTATION REQUIREMENTS:
  • Collect, report and document all applicable care and support information, using appropriate forms
  • Communicate changes in supports/services and training objectives in the service plan, as well as any significant changes/concerns in the individual’s life, to team members
  • Report and document allegations/observations of abuse, neglect and exploitation according to established policies and proceduresAbility to understand and implement behavior intervention plans, treatment plans, orders related to medications, individualized support plans and person-centered plans.

EDUCATION AND EXPERIENCE

High school diploma or GED equivalency is required. Must be 18 years of age or older. Valid driver’s license with clean driving record. May require training/testing to drive accessible vehicle. Prior experience working with children or adults with disabilities or challenging behaviors preferred. Successful completion and certification in training provided by agency within specific time allotment required.
