We have a number of exciting opportunities to join the Emergency Department Team at Tameside and Glossop Integrated Care NHS Foundation Trust as an ED Tracker.
The tracker role is a new role supporting both patients and clinical staff through their Tameside experience whether that be through the Emergency Department (ED) and Urgent Treatment Centre. (UTC)
The ideal candidate will be highly motivated, a good team player, organised, flexible and can maintain attention to detail. Enjoy working in a fast-paced environment, be adaptable and willing to take on various tasks and duties. The role will cover a 7 day service with shift patterns from 07:00 – 22:30.
You will also play an active role in the wider multidisciplinary teams that work across Tameside by liaising with different teams to ensure patients progress along their pathway safely and efficiently
The overall aim of this post is to offer the highest level of care to our patients by supporting the clinicians providing the front-line care to improve the patient experience and flow throughout the department.
Tracking patients from when they enter the department to when they leave by onwards referral for further specialist treatment or discharged back to the care of their GP in a timely manner. You will work closely with Medical, Nursing and Operational colleagues based in the ED and UTC department providing awareness to changes in pressure, capacity, and patient flow.

Section 1 - Main Responsibilities
Maintain the awareness to all staff in achieving the 4-hour access target for patients attending ED.
In collaboration with the ED shift co-ordinator and the Nursing and Medical team monitor closely ED patients and proactively manage those that have been in ED for more than 2 hours.
Attend Departmental safety huddles.
Be proactive in identifying and addressing delays in the patient journey.
Provide clear information to the shift leader/shop floor lead regarding delays in the patient’s journey.
Liaise with clinical speciality teams
Escalate any patient delays adhering to the Department/Trust escalation policy ensuring all the relevant parties have been informed.
Liaise with Bed Managers, speciality co-ordinators and ward staff regarding availability of beds.
Prepare for patient admission
Admit/scan/name band
Inform nurse when bed is allocated if there are delays escalate to shift coordinator.
Arrange transfer + / - escort
Liaise with Divisional managers of the day, Pathology, Radiology, Mental Health, PTS and social care to facilitate movement of patients within 4 hours.
Provide support regarding Lorenzo, E-card and Trust computer systems.
Undertake any administration/supporting activity that will shorten a patient’s time in ED, e.g. transport bloods to the lab, collect pharmacy products, scanning paper records, admitting patients, locating equipment.
Maintain accurate details on patients that breach the 4-hour target, completing 4 hours breach report live. Escalation of 12H breaches to ED Flow Matron/Ops management.
Manage all enquiries – in person or on the phone – in a prompt and professional manner.
Utilise all Trust computer systems efficiently to support departmental flow e.g. Lorenzo,
E-card, PACS
Follows Trust/departmental policies/procedures. Make any suggestions for improvement to work policies/processing own work area.
Responsible for inputting /maintaining patient data accessing one or more IT system in timely manner
Section 2 - Knowledge and Skills
Knowledge of Direct Attendance Recording and computer systems.
Good interpersonal and communication skills.
Section 3 - Effort and Environment
Requires sitting at PC for long periods, ability to transfer patients to all areas of the hospital and using the telephone.
Workload can be unpredictable and is managed rather than supervised
