Description:

An exciting opportunity has arisen for an Enhanced Clinical Practitioner to join our integrated frailty crisis pathway, which includes our Virtual Ward Team with a 2 hours urgent community response.
This is an exciting time to be part of service embarking on a transformation programme, developing new roles to lead, develop and deliver person-centered outcome based care to patients predominantly in their own homes.
We are looking for a highly skilled and experienced Nurse / AHP with a passion for working with frail older adults. Someone with expertise and a proven track record in driving innovation and enhanced practice to improve outcomes for our service users.
The post-holder will work closely with the consultants, advanced clinical practitioner and wider multidisciplinary team to serve Guildford and Waverley to deliver enhanced clinical care to our aging population, both in the community and in Care Homes to facilitate admission avoidance.
The post-holder will use expert knowledge and clinical reasoning skills to provide specialist support and aid decision making and plans for ongoing management. We are looking to recruit an exceptional attention to detail and accountable autonomous practitioner who is able to work proactively to monitor and anticipate changing needs of patients within a defined caseload and area.

The post holder is an experienced Nurse/AHP/Paramedic and will deliver autonomous patient care on a daily basis. The post holder is required to work at a standard where clinical and diagnostic skills are utilised competently and autonomously. The post holder will work in collaboration with the team to deliver a high quality service in order to meet the health and social care needs for patients attending the service.

2.1 JOB SUMMARY

Autonomously to assess, investigate, diagnose, treat and refer or discharge patients in their own place of residence within the parameters of the Enhanced Clinical Frailty Practitioner scope of practice. The post holder will be providing assessment, treatment, care and education to patients from all sections of the community, with a particular emphasis on older people living with frailty ensuring a high quality patient centered care. The post-holder may be required to work at any site where services are delivered by the Trust.
They will ideally meet the requirements for clinical knowledge and skills at an advanced level, but are not required to have a Masters in Advanced Clinical Practice.

Clinical

  • To ensure high quality, research based, individualised patient care is maintained at all times.
  • To clinically assess older people living with frailty using the Comprehensive Geriatric Assessment clinical model, including health assessment and ideally, clinical reasoning.
  • To clinically assess and triage as part of an integrated Urgent Community Response and Single Point of Access team, in to appropriate services within the broader integrated frailty pathway, including Same Day Emergency Care, Acute Frailty Units and Hospital @ Home.
  • To work flexibly across this integrated pathway as part of a cohort of practitioners.
  • To actively ensure the continuing implementation of changes to nursing / AHP practice to improve patient care utilising advanced skills.
  • To act as a role model and expert practitioner.
  • To ensure effective collaboration with all members of the multidisciplinary team and interested agencies.
  • To work within protocols set across Community Services for Enhanced Clinical Frailty Practitioners Providing advice, consultation and information about a range of health conditions and minor injuries/ailments.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations.
  • To actively participate in health promotion and education enabling patients to take responsibility for their own care.
  • To be aware of own limitations and limitations of the role, to refer to other practitioners when needs and risks are beyond their own scope of practice liaising with the multidisciplinary team as required.
  • According to current legislation and trust policy undertake independent, non-medical prescribing if relevant for the role.
  • Maintain legible and accurate patient notes, both written and electronic, in accordance with trust and national professional policies and guidelines.

Managerial

  • To implement and maintain Individualised Performance Review and objective setting for assigned staff, identifying training and development needs.
  • To ensure adequate clinical and managerial records are maintained.
  • To ensure that the environment and staff comply with Health & Safety, Infection Control & COSHH regulations and that Trust policies and procedures are followed correctly.
  • To promote excellent communications within the multidisciplinary team and to be responsible for appropriate communication structures being in place.
  • To ensure that incidents and accidents are reported and investigated according to Trust policy and that appropriate clinical risk management structures are in place.
  • To actively take part in clinical governance activities including governance meetings and Morbidity and Mortality meetings.
  • To encourage staff and maintain the highest standards of personal and professional conduct and performance, participating in the disciplinary procedure according to Trust policy.
  • To investigate complaints, prepare responses and implement necessary

actions as agreed within the directorate.

  • To be aware of the broader Trust wide issues and resources and work as part of the managerial team of the directorate.
  • To effectively manage the Single Point of Access and 2 hour response service as required ensuring that there is appropriate liaison with other departments and good utilisation of available resources.
  • To take lead in areas specific of practice within community services

Teaching and Training

  • To actively engage in the education and training offered through the Frailty Academy.
  • To ensure that the team provides a positive learning environment for

students, staff, patients and their families.

  • To ensure positive and appropriate links with the relevant University, particularly in regard to clinical placements and other continuous development opportunities.

To attend study/education for the service/individual need in accordance with the Trust policy, promoting professional development.

Professional

  • To maintain and develop one’s own professional knowledge and expertise and meet PREPP/revalidation and the job’s requirements.
  • To actively share knowledge and expertise and contribute to the Trust as a whole.
  • To reflect as a practitioner, learning from experience and to promote reflective practices within the ward/department team.
