Description:
Working as part of the Epilepsy Nursing Team this role will facilitate the co-ordination of care delivery, navigating and signposting patients to sources of advice and support.
It will involve close working with the CNS teams and regular liaison and communication with administrative teams including secretarial support, booking hub, community resources and many others. Responding to calls on the telephone helpline you will need to deal with simple enquiries and refer cases to the CNS Team as necessary.
There will be a considerable amount of preparation and organisation of notes.
You will provide information and support to enable people to navigate the health and social care system. A key aspect is communication with patients, relatives, carers and other health and social care professionals.
Applications are invited from individuals who have exceptional organisational skills and are able to work autonomously while building effective working relationships with others. You will need to be adaptable and capable of multitasking. Experience in working within health care service delivery will be an advantage. You will need to demonstrate a willingness to learn about Epilepsy and appropriate resources to support patients.

To provide effective and efficient administrative support to the Epilepsy Liaison Service, in particular:
1. Provide information and support about epilepsy and epilepsy services. The role will involve daily and direct telephone and email communication with patients, relatives, carers and other health and social care professionals, responding to queries and signposting where needed.
2. Co-ordinate access to information and educational resources.
3. Triage incoming calls and emails and initiate appropriate response according to protocols and individual pathways.
4. Coordinate the necessary assessments, appointments, referrals or investigations
5. Work alongside booking teams to enable effective referrals, booking and coordination of patient investigations
6. Liaise with nurse specialists/therapists, medical staff, clerical staff, community, primary care and ward staff and any other relevant agency key to ensuring the smooth pathway for efficiency and optimum patient care
7. Ensure that the patient and their carer are aware of the process to follow to make a complaint or compliment the service.
8. Accurately document all aspects of care co-ordination and service delivery.
9. Support the processing of referrals by ensuring that the service databases and Lorenzo system are updated with all admissions, transfers, discharges etc.
10. Arrange appointments and transport as required for patients.
11. To provide an element of administrative support to the specialist nursing team.

The Neurology Outreach Department provides a high quality clinical service to people with long term neurological conditions living in the community as well as those involved with their care/management within a defined catchment area.
There are currently five specialist service teams; Epilepsy, Multiple Sclerosis, Parkinson’s & Movement Disorder, Ataxia and Neuromuscular in the Outreach Department. The teams comprise of Specialist Nurses and Therapists who are predominantly supporting their patients with long term Neurological conditions to live fulfilling, lives at home.
The services offer a point of contact for care provision via various modalities including service specific telephone helplines, service email inbox, outpatient appointment reviews, home visits as well as day case assessments and contribute to inpatient care as required. The department works closely with Primary and Secondary Care colleagues as well as various other agencies to provide a coordinated holistic approach to our patients.
