DESCRIPTION

Excel Case Manager is responsible for the case management of each participant in the program, including social history assessments, provision and/or coordination of direct services, participation in team planning, utilization review, crisis intervention, and liaison services with referral sources. Excel Case Manager conducts support visits in participants’ homes and monitors progress through telephone and face-to-face contact. He/she prepares weekly reports on each client’s progress. Excel Case Manager also provides case management for adults charged with Contributing (non-school attendance).

QUALIFICATIONS

A valid Ohio driver’s license.
An associate’s degree (bachelor’s degree preferred) in Psychology, Sociology, Social Work, or a closely related field.
Three (3) years of relevant experience in a community behavioral health or other related field setting.

Must possess, acquire, and maintain the technical/professional expertise required to do the job effectively and to create solutions; display technical/professional expertise demonstrated through problem-solving, applying professional judgment, and competent performance; must be a team player, displaying respectful interaction with all other county personnel, and follow the employee manual and dress codes; ability to read, analyze, and interpret official reports, technical procedures, and governmental regulations; ability to write reports and correspondence; ability to effectively present information and respond to questions from other departmental staff, clients, professional peers in the community, and the public.

Responsibilities:

  • Meets parents and families wherever they may be with supports for self-management of health and well-being.
  • Conducts support visits in participants’ homes and monitors progress through telephone and face-to-face contact.
  • Demonstrates an ability to build relationships with participants and families.
  • Engages with participants using strengths based/motivational interviewing techniques to assess the needs of the participant.
  • Maintains professional relationships with peers, families, schools, and other local agencies.
  • Responds to urgent or emergency matters by phone or in-person as necessary.
  • Attends all Court staffings and status hearings for Excel and any other court hearings for participants as needed.
  • Coordinates services with community-based providers and maintains knowledge of community resources.
  • Supports participants in understanding of program expectations and progression.
  • Acts as a liaison between the Court, community resources, and treatment providers.
  • Maintains updated case notes and data in the Court’s database system on each participant, including notes of interactions with families.
  • Facilitates certain compliance and safety measures as needed including drug screening.
  • Assists Specialized Docket/Strategic Initiatives Deputy Director and/or Coordinator or other members of the management team as needed with tasks, assignments and projects related to the program or other matters within the Court.
  • Ensures that all required paperwork is completed on time and maintained in confidential files.
  • Prepares detailed status reports for each participant and submits to Court as instructed.
  • Makes community based/home visits to each participant as needed by policy and directive.
  • Notifies parties of relevant issues pertaining to the participant, i.e. new use, dangerous behaviors, child risk, change in living arrangements, intimate partner violence, criminal charges.
  • Contacts collateral sources/service providers on a regular basis.
  • Assists in planning incentives and commencement activities.
  • Organizes and facilitates post-commencement/alumni group.
  • Demonstrates regular and reliable attendance.
