JOB DESCRIPTION
You will assist the Medical Social Worker in assessing, evaluating, planning, coordinating resources and services for the continuity of care for the residents & collaborate with multidisciplinary health service professionals to develop care programs for timely treatment or recovery.
- Monitoring of Bed Occupancy Rate and meeting admission KPIs on a monthly basis
- Keeping track and managing the residents’ data and information
- Ensuring proper filing of residents’ data and information
- Applying financial schemes and monitoring the payouts for residents on a monthly basis
- Monitoring and ensuring that residents’ Household Means-Test is renewed before expiration
- Preparing Medifund reports and documents to present to Medifund Committee
- Answering and addressing queries and concerns from residents and their next-of-kin
- Any other case management work as assigned
QUALIFICATIONS
- Diploma in Nursing, Social Work or other related disciplines
- Minimum 1 year experience in community healthcare settings in Singapore
- Good knowledge of the geriatrics and community services
- Strong communication skills (oral and written)
- Proficiency in G Suite applications (e.g. Google mail, docs, sheets, slides, calendar)
