Responsibilities:

ROLES & RESPONSIBIITIES

Outreach
Plan, execute and evaluate outreach programs for the community ( e.g. mental health outreach, preventive and developmental programmes/workshops in schools and the community etc)

Case Management

  • Conduct mental health screening, risk and needs assessments by taking into account the integration of a range of information, which includes appropriate administration of mental health assessment tools and other relevant frameworks
  • Create, implement and review case plans which are reflective of proper case assessment/ conceptualization and intervention
  • Conduct Basic Emotional Support for clients.

Groupwork

  • Plan and facilitate group interventions independently, which includes facilitating case conferences with significant others and relevant partners.
  • Evaluate and review group interventions for the improvement of programme/s

Professional Practice

  • Demonstrate professional practice and role in accordance to the organization’s vision, mission and best practice standards of the sector, which include ethical, reflexive and collaborative practice
  • Respond to crisis situations based on organisational standard operating procedures
  • Identify areas of improvement in service delivery based on evidence-based practice
