Description:
- Establishes contact with new members and 1) ensures enrollment in text program and MyChart and 2) ensures first visit is scheduled.
- Reviews charts prior to wellness visits (annual wellness visits/executive wellness visits) and prepares as appropriate, including but not limited to, ordering labs, contacting patients to coordinate ancillary services, coordinating care with provider, etc.
- Schedules appointment(s) and/or procedure(s) with specialist(s) at request of ordering provider.
- Ensures members’ clinical needs such as patient advice requests, prior authorizations, etc. are promptly completed.
- Ensures staff properly implement and adhere to clinical policies and procedures.
- Ensures patient care is delivered in a safe manner.
- Ensures required reports and logs are completed timely and maintained per policy.
- Maintains inventory of all medical supplies.
- Identifies ways of improving current services with medical director and clinic leadership.
- Continually monitors operations related to clinical care and regulatory requirements and recommends policy, process, and procedural changes when appropriate.
- Keeps medical director and clinic leadership informed of activities, needs, and problems to promote clinical efficiency and effectiveness within the clinic.
- Recommends and oversees the development of approved performance improvement initiatives.
- Collaborates with medical director and providers regarding clinical aspects of patient care to ensure the provision of high-quality health care services.
- Ensures medical equipment is operational and clinical areas are clean and well-maintained.
- Assists clinic leadership with the selection, development, and motivation of clinic staff.Provides guidance to others regarding clinical practice, serving as a preceptor, mentor, and resource.
Responsibilities:
- Presents a professional image in conduct, attitude, and attire.
- Assists on-call provider with 24/7 phone triage coverage as requested
- Performs all other clinically relevant duties as assigned.
Qualifications
Experience: 3 years of nursing leadership or 5 years of care coordination
Education: Bachelor’s Degree in nursing
Special Skills: Ability to communicate with patients promptly, assess their needs, and triage as appropriate. Leadership skills demonstrated in decision making and problem solving.
Licensure: Valid Louisiana Registered Nurse License
