JOB DESCRIPTION
At Assist Home Care Ltd, we are looking to recruit an Experienced Care Manager to join our expanding team. We are seeking a dedicated, enthusiastic, and compassionate individual to support our growing team of care and support workers. We provide services to both adults and children across Bradford, Kirklees, and Calderdale, particularly supporting individuals with a variety of complex care needs.
We are looking for an individual with extensive experience in Care, Complex Care, and Learning Disabilities settings. The successful candidate will have a proven track record of managing care teams at a senior level, ensuring the highest standards of service delivery and compliance with all regulations and policies.
KEY REQUIREMENTS
- Extensive experience in Care, Complex Care, and Learning Disabilities settings.
- Proven track record of managing care services, ensuring CQC compliance and delivering high-quality care.
- Strong people management and coaching abilities, with the ability to motivate and guide teams effectively.
- Experience in rota management and resource allocation within a care environment.
- Knowledge and experience of supporting individuals with a variety of learning disabilities and complex care needs, including conditions like autism, cerebral palsy, epilepsy, PEG feeding, Down syndrome, and neurological conditions.
- Excellent communication skills, with the ability to liaise with service users, families, healthcare professionals, and internal teams.
- Experience in clinical reviews and developing comprehensive care plans.
- A valid driving licence and access to a car (required).

Responsibilities:
- Oversee the delivery of care services for adults, ensuring that high levels of care are provided in line with regulations, alongside the Registered Manager (RM).
- Ensure strict compliance with CQC standards and internal policies to guarantee quality care.
- Manage staff effectively, including rotas, resource allocation, and fostering a collaborative, supportive work environment.
- Develop and implement comprehensive care plans tailored to the needs of each service user.
- Liaise with families, healthcare professionals, and multidisciplinary teams to ensure coordinated and holistic care.
- Conduct regular assessments, supervisions, and overall management of support workers, providing guidance, feedback, and development opportunities.
- Ensure that all policies, procedures, and clinical reviews are followed and adhered to at all times.
- Take a leading role in onboarding and training new care workers and team members to uphold service standards.
- Drive collaboration across departments such as compliance, training, and recruitment, ensuring smooth operations and effective work practices.
- Serve as the main point of contact for operational needs, providing support and resolving challenges promptly.
- Monitor live calls using custom software, ensuring efficient care delivery and effective communication within the team.
- Oversee the rotas and ensure that care is delivered to service users as per their individual care agreements and contract specifications.
- Support care package implementations, ensuring your team is prepared and equipped to deliver the high standards expected.
- Foster a culture of continuous professional development, actively seeking opportunities for both your growth and that of your team.
- Provide support with on-call responsibilities, offering guidance and solutions during out-of-hours situations as required.
