Description:

An exciting opportunity has arisen for a Band 6 Occupational Therapist to join our Cardiac Rehabilitation Team in Carmarthenshire.
Delivering services for patients closer to home in line with the Health Board’s strategy ‘A Healthier Mid and West Wales: Our Future Generations Living Well’ and associated plans. Cardiac Rehabilitation aims on delivering an effective, efficient, and sustainable service by shifting the current focus on hospital-based care and treatment to a focus on services more readily available in local communities.

We’re seeking to appoint a proactive, motivated and compassionate individual with demonstrable experience, whose values are aligned to those of Hywel Dda University Health Board:

Operate as an autonomous practitioner, with a varied and often complex caseload, using specialist knowledge of occupational therapy and the functional impact of health conditions to appropriately select, plan, modify, carry out and evaluate assessments and interventions, adapting practice based on client’s circumstances and presentation.
Carry out group education (fatigue/stress/anxiety management) with individuals both virtually and in-person.
Work effectively as part of a multidisciplinary team to provide rehabilitation/recovery focussed on the needs of the individual.
Works independently or alongside other members of the team, where relevant, to plan, organise and deliver therapeutic interventions with individuals or groups, across a variety of settings to include hospitals, person’s home, educational establishments, and community & leisure facilities.
Advises/educates and demonstrates specific techniques/strategies to the individual and their families/carers, if relevant, to support the ongoing management of the individual’s recovery.
Delegate clinical work to others within the team i.e. Support Workers and students, following relevant policies/guidelines.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;
Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;
48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
Numerous locations providing mental health and learning disabilities services;
Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
When an individual suffers from a cardiovascular disease, it may have a significant impact on their physical and psychological health. Patients who have cardiovascular disease may experience fatigue, shortness of breath, depression, anxiety, memory and concentration problems, and difficulty participating in everyday activities. Cardiovascular disease is a serious and often emotionally distressing condition that can result in functional limitations.
This post offers an excellent opportunity to test, trial and/or build upon existing clinical and leadership skills within the clinical speciality. Training, role orientation and ongoing support will be provided. The successful candidate will have access to regular clinical and managerial supervision.
The post holder will have current HCPC registration. They will be expected to travel within Carmarthenshire and at times with the possibility of the wider Hywel Dda University Health Board area, There are opportunity for pool cars and mileage expenses.
