Salary: £40,136 - £42,125 (Inclusive of 7.5% market supplement)
Hours per week: 37 hours

Join our expanding Continuing Health Care Team and become an integral part of a dedicated group that supports individuals with health and social care needs across the county.
As a Social Worker in our team, you’ll have the unique opportunity to combine leadership with skill development, directly impacting the lives of adults requiring continuing health care assessments. You’ll be at the forefront of person-centred practice, where you’ll assess, plan and manage risks for individuals, helping them navigate their health and social care journeys with dignity and support.
This role allows you to tackle complex challenges by undertaking Continuing health care assessments, supporting carers and applying Council policies to empower individuals to manage risks effectively.
It’s a role that not only challenges you but also offers opportunities for growth and continuous learning, all within a supportive and dynamic team environment.
We’re looking for a qualified Social Worker with at least two years of post-qualification experience in a social care setting. The ideal candidate will bring extensive knowledge of health and social care legislation, policies and guidance, along with a proven ability to craft clear, complex and accurate reports. Due to Wiltshire’s rural nature, a full driving license and access to a car are essential.
