Our caring team provides education and support for managing chronic diseases and mental health concerns, while providing access to specialists and services all within your local community.
We are currently looking for a Primary Care Family Practice Nurse - LPN, to join our team at clinics in Gibbons, AB. This is a great opportunity to work directly with family physicians in a Primary Care setting and work to your full scope of practice.
- Permanent 0.8 FTE
- 31 hours/week (Monday-Thursday 8:45AM-5:00PM)

POSITION SUMMARY
The role of the Sherwood Park – Strathcona County PCN Family Practice Nurse - LPN is to provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The nurse will work collaboratively with patients, family physicians and other team members to develop and implement care plans and provide follow-up with patients. The nurse will assess, manage and support patients to provide comprehensive care, education and guidance to patients with a variety of clinical conditions. Duties will vary depending on the clinic and patient population.
EDUCATION & EXPERIENCE
- Licensed Practical Nurse Diploma required
- Three years related nursing experience, Primary Care environment preferred
- Current CLPNA Registration, CPR, and BCLS
- Experience working in a team setting with other health professionals
- Experience in establishing protocols for program startups, especially developing plans and priorities when workload varies
- Experience in developing plans and priorities with variable workloads
- Experience working in a team setting with other health professionals
SKILLS & ABILITIES
- Strong knowledge of nursing processes and practices, including intervention strategies and therapies
- Strong, effective written and oral communication skills, high level of perceptiveness
- Ability to prioritize, manage time effectively and be flexible in a very active work environment

OTHER OCCUPATIONAL REQUIREMENTS
- A current and clear Police Information/Vulnerable Sector check is required
