QUALIFICATIONS

• Associate Degree in a healthcare related field or Bachelor’s Degree required
• Creighton FertilityCare Practitioner Certificate required
• If not yet certified, completion of Education Phase 1 Accredited Creighton Model FertlityCare Practitioner Program required (Must be in good standing with Program Educator.)
