Description:
The post holder will provide care for patients within the Primary Care Network (DN PCN). We are a group of 11 practices covering a population of 58,000. Our current priorities are:
- Improving Access to Care.
- Improving Cancer Diagnostic Pathways
- Enhanced Health in Care Homes
You will work in 2 practices within the PCN (Evehill and Central Clinic) – which will give you variety through the working week. In each practice you will have support from a named GP who will be available to review decisions when you feel this is wanted.
You will work collaboratively with GPs and other healthcare professionals to ensure optimal end-of-life care for patients in the community.
This is also a development role. We will provide training opportunities to make sure that you are growing in your skills. Eventually we want you to be leading the next generation of Paramedics into Primary Care. As a PCN we are actively looking to encourage Paramedics to get trained up to an ACP/ECP.
As a Paramedic in Primary Care you will be acting within your competency to make diagnoses and plan management. You will always have appropriate medical supervision. We want you to feel at home – to work safely, to learn and develop, and to make a real contribution to the healthcare system in our area. We will ensure that your CPD is supported in line with your educational goals.
Paramedics will be expected to work autonomously within primary care at an advanced level of practice, using their enhanced clinical assessment and treatment skills, to provide ongoing and coordinated care to the most vulnerable patients with complex needs within the PCN.
The paramedic will have the capability to make sound judgements in the absence of full information and to manage varying degrees of risk when there is complex, competing or ambiguous information or uncertainty. They will work closely with the wider MDT to deliver joined up care, referring to other specialist providers and services when necessary.
Here at the Dudley Group our patients and staff are at the heart of all that we do to offer a high-quality patient experience in a caring and supportive environment that aligns with our vision of providing safe, caring, and effective services because people matter.
