JOB SUMMARY

We are excited to be recruiting a Consultant Practitioner in Frailty to work alongside our existing Consultant Workforce at Urgent Community Response and Frailty H@H Virtual Ward.
The Team operates across Mid Hampshire to deliver acute hospital level care at home in response to urgent changes in health status for patients living with frailty.
This vacancy is a direct result of our success and the recent expansion of the service through increased commissioning and a very successful GIRFT review.
The successful applicant will be joining an established and supportive group of Consultant Practitioners from varied backgrounds. We welcome applications from all Allied Health Professions and Nursing. The successful applicant will be supported to develop their Multi-Professional Consultant Practice Portfolio working towards the ‘Expert’ capabilities.

Responsibilities:

The post holder will be required to work autonomously. The foundation nature of the post means appropriate supervision from Consultant Practitioner is available to support the Foundation Consultant Practitioner to develop and work towards the competencies expected of a full Consultant Practitioner. These include;
Establish values based professional expert and evidence-based practice across pathways, services, organisations, and system working with individuals, families, carers, communities, and others.
Provide values-based leadership across the care pathway, services, and systems in complex and changing situations.
Develop staff potential, add to, and transform the workforce, and help people to learn, develop and improve (in and from practice) to promote excellence.
Develop a ‘knowledge-rich and inquiry’ culture across the service and system that contributes to research outputs and has a positive effect on development, quality, innovation, increasing capacity and capability, and making systems more effective.
Establishing expertise across the system and nationally by using consultancy approaches and opportunities that have maximum impact on practice, services, communities, and populations, and which add to and sustain workforce capacity and capability.
