Right at Home Sydney Inner West are seeking a qualified and experienced Care Coordinator (Full Time) for our in-home care company. Our suitable candidate will demonstrate client-centred practice, showing empathy, and treating others with dignity and respect. You’ll be a natural relationship builder, honest, helpful, and dependable; making you a great asset to our team.

• Meeting with clients and assessing their needs.
• Preparing in-home care assessments and care plans to best meet client requirements.
• Provision of in-home care services as needed.
• Handling information requests, feedback, general enquiries and complaints.
• Training, and managing the performance of the care team.
• Ensure all documentation is accurately and comprehensively completed in Right at Home databases and software programs.
• Providing after-hours “on-call” phone services on rotating roster.
