QUALIFICATIONS AND REQUIREMENTS
We seek candidates who meet the following qualifications and requirements:
- Graduated with a Degree or Diploma in Behavioural Science with an emphasis on Social Work, Counselling or Psychiatric Nursing; equivalencies may be considered
- Experience with clients with a history of conflict with the law, developmental disabilities and/or spectrum disorders, interaction with social agencies and experiencing psychiatric/personality disorders
- A demonstrated and clear ability to respond well in crisis situations
- Understanding of providing service to people with substance abuse challenges
- Experience in assessment, case/care plan development and implementation.
- Demonstrated an ability to deal with potential violence
- Demonstrated an ability to develop programs tailored to the particular diverse needs of the individual
- Excellent written and verbal communication skills in English
- Valid class 5 driver’s license
- Personal transportation in good repair, insured for business up to $1,000,000 for liability, and suitable for client transport
- Demonstrated ability to respond efficiently in crisis and escalated situations
- Diploma in Social Services and one year of relevant experience, we will consider a relevant combination of education and experience
- A background in professional or lived experience working with vulnerable populations facing multiple barriers
- Successful completion of a Vulnerable Persons Criminal Records Check
- Current Emergency First Aid Certificate
- Proof of Covid-19 Vaccination
Responsibilities:
ROLE PURPOSE
Our Outreach and Community Inclusion programs help individuals access services and build community connections, while supporting them to develop life skills. We work one-on-one with service users to encourage them to identify, work toward, and achieve their goals so they can build greater independence and more active community engagement. Community-based outreach supports may include employment, housing, building healthy relationships and pursuing social/recreational opportunities.
This position will provide assistive outreach support to residents of the Connective Support Society’s programs referred by Community Living British Columbia to enable their adjustment to, and success with life in the community.
AS AN OUTREACH WORKER, YOUR RESPONSIBILITIES WILL INCLUDE:
- Works with the client, researches background material, and seeks the input of other professionals and family members, to provide the assessment that will ascertain needs and postulate intervention possibilities
- Works with the client to consider issues and creates a mutually agreed intervention and support plan that has measurable short and long-term goals
- Ensures issues of daily living, including but not limited to housing, nutrition, clothing, hygiene, leisure lifestyle and access, employment, and education are addressed in this plan
- Ensures the client’s ongoing legal, mental, and physical health issues are addressed in the plan
- When and as needed, provides a wide range of interventions to enable the client to address and accomplish the established goals
- Interacts with other professionals, family members, and organizations to ensure services, admissions, and treatments are in the client’s interest and supportive of the long-term goals
- On the client’s behalf, is a resource to other professionals, including but not limited to the courts, probation services, and community and institutional psychiatric services
