Full Time Case Manager - ICMS Essex

Mount Carmel Guild Behavioral Health System, a CARF accredited division of Catholic Charities of the Archdiocese of Newark, is a leading provider of mental health and addiction services in Northern New Jersey.
We are currently seeking a Full Time Case Manager for our ICMS - Essex program located in Newark, NJ. (Schedule: M-F 8:00AM-5:30PM, with possible Saturday hours.)
The Integrated Case Management Services program (ICMS) provides “in vivo” case management services to adults with a severe and persistent mental illness with co-occurring substance abuse. Services are designed to engage, support, and link clients to traditional and non-traditional recovery oriented supports and services. The goal is to maximize clients’ independence by providing personalized, flexile, and collaborative services in the clients’ natural environments.

REQUIREMENTS

• Bachelor’s Degree in a behavioral health discipline
• 1-2 years of related experience
• valid Driver’s License
  Preferred experience includes one year of experience in a behavioral health setting. Bilingual preferred.

• To maintain a caseload of up to 25 consumers and provide linkage and referral services to adults with severe and persistent mental illness in the community.
• To collaborate with the consumer’s family, community, and agencies to ensure the consumer is working towards goals of self-sufficiency.
• To maintain electronic health records in compliance with state regulations.
• To provide 50% of direct consumer contact via telehealth and in person visits.
