WHAT YOU WILL DO…

As a Case Manager you will support highly vulnerable newcomer clients/families during their settlement process by providing wrap-around client-centred case management services (in person and/or virtually). A Case Manager is responsible for providing one-on-one and/or family case management support and group services to newcomer population who struggle through multiple barriers during their settlement process, with the goal of building and enhancing their capacity towards self-independence and self-reliance. Through outreach you will connect with eligible newcomer clients with complex needs and multiple barriers that could prevent them from successfully settling in Canada. The services that you will be offering to the client include information, orientation and appropriate referrals to community and government services.You will conduct a needs assessment to identify client’s needs and create a client-centred wrap-around service/settlement plan based on these needs. You will act as an advocate in helping newcomers access services including education, health care, legal systems and provide support in completed processes to access these services. You will provide case conferencing, supported access and transition support to external services where needed. You will also provide continuous case monitoring and follow up to support future settlement needs You will enter information into government/ department databases and prepare monthly narrative and statistical reports for the Program Lead. To further support client and program needs, you may be asked to facilitate group or individual sessions, and connect with our community partners as continuous work to ease navigation and increase access of the settlement service system in our community. Other duties may be assigned based on client and program needs.
