Description:
QUALIFICATIONS:
Education/Experience:
- Current unrestricted certificate of registration with the Ontario College of Nurses
- Current BLS certification
- Minimum of five (5) years’ experience working within Specialized Geriatric Services and/or with geriatric population
- Completing assessment and treatment guided by the Comprehensive Geriatric Assessment.
- Advanced knowledge, skills, understanding of best practices related to dementia, delirium, mental health and its effect on the person and family members/care partners
Skills/Abilities:
- Excellent critical thinking, interpersonal, diplomatic and communication skills (verbal and written)
- Effective listening, observation and facilitation skills to effectively assess the client and provide appropriate treatment and consultation
- Ability to function and contribute as an effective team member and to work independently
- Excellent problem recognition skills with the ability to proactively achieve resolutions both through direction and negotiation and make decisions rapidly
- Demonstrated knowledge, understanding, and commitment to client-centred care
- Experience in providing focused geriatric assessments using various tools in order to identify high risk geriatric patients
- Knowledge of community and regional services/resources for the geriatric population
- Demonstrated leadership experience and collaborative leadership style
- Demonstrated discharge planning experience
- Established networks with community/regional partners who specialize in servicing geriatric populations
- Demonstrated adherence to PFCC philosophy, knowledge of CNO standards of practice
- Demonstrated leadership and participation in patient flow initiatives
- Demonstrated ability to work under pressure and manage competing work demands
- Proficient computer skills in Meditech, Strata, Excel, Word & Power Point programs
- Demonstrated regular attendance at work
Assets:
- CNA Certificate in Geriatric Nursing/Rehab Nursing
- Membership in professional association e.g. RNAO
- Experience working with Indigenous persons and First Nations
- Knowledge of multi-cultural beliefs, values and perspectives with particular emphasis on Indigenous persons and First Nations
