Description:

Clinical responsibilities: The post-holder will be required to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing/authorising repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation. Assessing the health care needs of patients with undifferentiated and undiagnosed problems. Screening patients for disease risk factors and early signs of illness in consultation with patients and in line with current practice disease management protocols, developing care plans for health.
Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate. Recording clear and concise consultation notes Collecting data for audit purposes. Prescribing in accordance with local formulary and national guidelines The post-holder may be required to visit patients during surgery time in an emergency if clinically indicated. The post-holder may be required to support enhanced access sessions as per the needs of the PCN.
Communication Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment. Recognise the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating. Delivering a quality service Prioritise, organise and manage own workload in a manner that maintains and promotes quality, including participating in shared learning across the practice, PCN and wider organisation. Work as part of the team to seek feedback, continually improve the service and contribute to business planning.
Learning and development Take responsibility for own learning and performance through continuous professional development, including participating in supervision, acting as a positive role model and assessing own learning needs. Act as a mentor and positive role model to students and junior members of staff, sharing information and good practice. Contribute to education and training of trainee clinical staff and students. Maintaining confidentiality The GP will, as an integral part of and in the course of carrying out his/her role, have access to confidential and sometimes sensitive information relating to patients, carers and family members, as well as similar information about colleagues.
They may also have access to confidential information about the practices and the PCN. All such information, in whatever format and howsoever made available, must be treated with the strictest confidence. All such information held within and outside of the practice will be shared only as necessary, and in accordance with the practices confidentiality / data sharing policies, and in line with data protection legislation and the Freedom of Information Act.
