Description:
The Multi Agency Care & Coordination (MACC) Team is a recently established and developing preventative service for Haringey adults living with frailty or multiple long-term health and social care needs. The MACC service is a core element of Haringeys Anticipatory Care plan. Following an anticipatory care model, we provide structured proactive care and MDT support to patients with rising risk before they require acute interventions. Key aims: 1.
To benefit patients with multiple needs, and their carers, by enabling them to stay healthier for longer, maintain or improve functional ability and benefit from positive experiences of proactive, personalised, and self-supported care. 2. To reduce need for reactive health care and supporting actions to address wider determinants of health. To deliver better interconnectedness between all parts of the health system and the voluntary and social care sectors
