Description:
TOTAL PROGRAMMED ACTIVITIES: 4 There will be no on call duties for this post Main duties of the job Leading our teams to further develop and deliver a comprehensive 7-day Frailty & Community Based Service. Promoting a culture of proactive decision making, maintaining independence and of keeping patients living safely at home. To support the prevention agenda for frailty supporting patient independence To provide clinical advice & support to primary care colleagues and other professional across the integrated model of care To support the roll out of proactive community based assessment, frailty screening and Comprehensive Geriatric Assessment. To provide review of patients triggering further assessment following CGA To provide medical support for patients identified as needing specialty frailty review from falls screening programme.
To support the model of community-based rehabilitation (Community Hospital, Step down/Step up facility and Reablement at Home) To lead in liaising with neighbouring Health Boards and Trust across Wales and England to develop shared pathways and to promote and grow the model of early Discharge to Recover and Assess To help scope and develop Same Day Emergency Care for elderly patients building on the existing models of the virtual wards. To help to develop community-based/virtual outpatient review To take part in management and audit activities to support the planning for services and ensuring the service meets key performance indications including excellent patient care. To support PTHBs research portfolio through active participation in projects led by colleagues (internal and external) To support the teaching and training deliver training across Primary and Community care multi-disciplinary teams The post will be subject to the terms and conditions of service governing the appointment of Medical and Dental Staff in Wales, and to the Health Board’s terms and conditions as appropriate. The candidate(s) to be appointed will be required to be fully registered with the General Medical Council and to have higher training in frailty medicine
