THIS POST IS FIXED TERM/SECONDMENT FOR 5 MONTHS DUE TO FUNDING

We are looking to appoint a dynamic-forward thinking General Practitioner with Extended Role (formerly a General Practitioner with Special Interest) in Community Frailty medicine with a keen interest in service development to help to lead the development of our community frailty services, providing a patient centred system wide proactive and flexible service for patients living with frailty in Powys.
Our effective frailty model of care is based upon early recognition and prevention – proactive rather than reactive care. Our ultimate aim is to help our patients to live healthily and independently at home with their families and communities.
This new post provides an exciting opportunity to help lead and support our programme of service transformation building on existing established models of care, providing clinical support for the community teams across the community hospitals and the virtual wards.
The provisional interview date is likely to be 14/11/2024
The post will be subject to the terms and conditions of service governing the appointment of Medical and Dental Staff in Wales, and to the Health Board’s terms and conditions as appropriate. The candidate(s) to be appointed will be required to be fully registered with the General Medical Council and to have higher training in frailty medicine.

Responsibilities:

  • Leading our teams to further develop and deliver a comprehensive 7-day Frailty & Community Based Service.
  • Promoting a culture of proactive decision making, maintaining independence and of keeping patients living safely at home.
  • To support the prevention agenda for frailty supporting patient independence
  • To provide clinical advice & support to primary care colleagues and other professional across the integrated model of care
  • To support the roll out of proactive community based assessment, frailty screening and Comprehensive Geriatric Assessment.
  • To provide review of patients triggering further assessment following CGA
