Description:
Harrow Training Hub The role of Training Hubs is to: Have a key role in understanding, influencing, supporting, and leading educational interventions around population health needs and health inequalities in their area to address these. Provide advice on workforce planning and training needs analysis, to help find which roles best meet the needs of the population. For example, actively supporting workforce planning by understanding population health needs and the training needs required to support new care pathways. Help new staff, appointed through the Additional Roles Reimbursement Scheme (ARRS) scheme, embed into roles, for example supporting the roll out of the First Contact Practitioner (FCP) programme.
Facilitate and/or deliver opportunities for Continuing Professional Development (CPD) programmes, based on scoping CPD requirements across PCNs. Provide career support at all stages for all primary care workforce. Train and, where appropriate, recruit more educators. Develop and help to keep staff through retention programmes, for example mentoring, and preceptorships.
Support practices and PCNs, who are looking to become learning environments, to increase the number of placements for a variety of trainees and students. KEY RESPONSIBILITIES: Understanding of primary care workforce requirements and workforce transformation initiatives within the NHS and the current healthcare environment, to ensure pertinent involvement from the Training Hub and appropriate development of education and training programmes. The post holder will support the on-going development of the Training Hub, in collaboration with NHS organisations, community providers, as well as local authorities; ensuring that there is a clear identification of local requirements and plans in place to recruit and develop well trained staff to meet those needs. Work closely with Training Hub colleagues across NWL to ensure the sharing of good practice and opportunities of working at scale are fully explored.
The post holder will carry out any other duties as may reasonably be required by their line manager. The job description and person specification may be reviewed on an ongoing basis in accordance with the changing needs of the programme and the borough. Key Activities Provide operational and management support for a range of team initiatives, including but not limited to support of the training hub and PCN support including meetings and administrative duties. Developing and maintaining effective electronic and paper filing systems, to ensure that information is kept securely and is accessible to other members of the team.
Support the delivery of the primary care workforce programme and initiatives in Harrow, linking with NHS organisations, Community Services, Social Care, Higher Education Institutions and Schools. Support programme elements including: workforce retention, placements, supervision, competency frameworks, developing career pathways, broadening skills mix, integrating roles and leadership development. Support the training hub lead to undertake an annual Training Needs Analysis of the training development requirements of practice staff that will inform the future commissioning of education provider services. Identifying, reviewing and benchmarking against best practice and national workforce programmes.
Support the Training Hub to improve its maturity and delivery of the Health Education England and local KPI targets and governance requirements. Facilitate improved use of information systems and processes among member practices, supporting improved submission of data and promoting enhanced data quality. Providing administrative support to local meetings and represent Harrow TH at NWL wide meetings, providing reports on attended meetings. Help with the response to invitations to tender for provision of training and education opportunities or innovative programmes of work relating to the hub core function.
Mapping out training plans and schedules. Work with colleagues linked to Training Hubs across NWL to identify opportunities to deliver collective/shared learning. Organise and plan training events, forums, webinars and meetings, including the production of supporting information. Meetings and training events will include PCN and individual Practice meetings, plus other topic-specific events.
Preparing agendas and papers, taking minutes and distributing notes of meetings, including typing up of group discussions and interviews as necessary. Plan, manage and prioritise own workload ensuring all deadlines are met and complying with key performance indicators. CRM Hub Pilot KEY RESPONSIBILITIES Support meetings for the development of the CRM Hub pilot, to drive patient outcomes, long-term interventions and ensure successful care planning for patients. Act as the senior point of contact for stakeholder engagement on behalf of General Practice Work collaboratively with and develop the CRM hub in partnership with Harrow PCNs, working dynamically with General Practice, responsively building iterative ways of working that are responsive and pragmatic with an understand of Primary Care priorities Engage with the CRM Hub meetings/forums e.g., MDT, steering group, and ad-hoc meetings.
Help develop performance metrics for the pilot and project planning goals with the Clinical Lead. Support the Clinical Lead ensuring that all clinical practices and protocols are being followed, and working closely with other healthcare professionals to ensure the smooth running of the Hub activities. Develop the core team to support processes for referrals and evaluation. Build and maintain dynamic relationships with Primary Care, Acute, community and the voluntary sector.
Advocate for social prescribing and community incentives To support the evaluation on service activity and service quality both quantitative and qualitative as required in partnership with project stakeholders. Support the conducting of research to identify areas for improvement. Assist with the evaluation of clinical outcomes to identify areas for improvement. Ensure that they demonstrate and maintain high levels of good medical practice, mandatory training requirements and reflective practice.
Establish and maintain processes for managing scope during the project lifecycle, setting quality and performance standards and assessing risks. Develop and maintain partnerships with stakeholders and third parties. Assign and monitor resources to ensure project efficiency and maximize deliverables. Report project outcomes and/or risks to the appropriate management channels and escalate issues, as necessary, according to project work plan Service Innovation, Improvement and Development To support and lead the development of Harrow Network Partners and innovation by using own knowledge, experience and broad range of networks and contacts to share learning and ideas.
Take the lead for creating and developing the long-term strategic plan for the company. To promote and develop services in a way which leads to increased population health. To support the establishment of processes and project management structures for agreeing and delivering new care pathways. Support the Directors in ensuring a robust Corporate Governance.
Be responsible for managing company finances ensuring they are within any budgetary constraints and all expected income is received. To ensure financial risks are recognised and appropriate action taken to develop and champion robust mechanisms for ensuring learning from patient experience and public engagement is incorporated into service redesign. Lead and manage effective bidding processes including benefits realisation and impact assessment for service and business development opportunities, ensuring key clinical staff are engaged and contributing effectively. Ensure that meaningful systems for involving service users, staff and the public in business and service development are established.
Undertake analysis of performance data and capacity and demand data to identify areas for service redesign and collaborative working. Present complex, sensitive and highly contentious information and solutions to the executive, managers and teams in a manner that generates understanding and support, having analysed the evidence and options. Develop relationships with current and potential partners to progress mutually beneficial initiatives and service developments. Promote positive working relationships with other services and other organisations and agencies externally.
Maintain an up-to-date knowledge of and apply all new directives, health services guidelines, advance letters and any legislation that will impact upon service developments To lead the resolution of any complaints, resolving informal complaints locally and assisting in the investigation of and response to formal complaints as required Communication The post holder will be required to communicate and provide complex information to a wide range of internal and external stakeholders. Represent Primary Care and advocate for its role in the NHS and fair representation and resources. Participate in relevant internal and external working groups/projects, and initiatives which may be highly complex, sensitive, political and contain contentious information with the aim of providing information an analytical advice to the team. Work with members of the team to develop and implement project data collection systems that will provide accurate and timely data.
