Responsibilities:

MAIN PURPOSE:

  • To take responsibility for the health care needs of individuals in the service ensuring the highest standards of care are maintained and delivered.
  • To lead, develop, direct, and supervise the nursing team and Rehabilitation Support team to support, guide and enable people we support with brain injury in the performance of daily living functions and the development of personal skills and potential.
  • To support the team in meeting compliance in line with requirements of the regulatory body and relevant legislation.
  • To lead assessments for individual referrals and people we support ensuring information is communicated effectively.
  • To lead on health promotion for people we support in the service.
    To support the service manager in leading the ongoing development of service provision, through development, delivery and maintenance of high standards of evidence based clinical care and safety

MAIN TASKS AND RESPONSIBILITIES:

  • Assists the Service Manager in the continuing responsibility for the management of the unit.
  • Demonstrate professional leadership and provide ‘hands on’ support and guidance, managing staff activities and influencing their practices and attitudes
  • Assume line management responsibilities for the nurse /support worker team (delete as necessary), to provide operational support and direction. Ensuring that performance standards are identified and met.
  • To support and coordinate assessment of referrals and plan and organise admissions and discharge of people we support to the unit through coordination and review
  • To lead on agreed Quality Assurance Audits and Clinical Governance, ensuring relevant documentation is completed by self and according to Brainkind policy and relevant legislation, and that episodes of care and treatment are evaluated and reviewed according to individual requirements
  • Deputises for the Service Manager in his/her absence as appropriate
  • Lead and oversee the nurse/support worker team in delivering the continuum of care planned for the individual, ensuring involvement with people we support, carers and families provided
  • Lead on the assessment, planning, delivery, evaluation and review of indivualised evidence-based care and support, coordinating and training other staff to ensure delivery of planned care and using outcome measures to monitor effectiveness.
  • Lead and advise on the health care management of individuals ensuring health promotion advice and opportunity is accessible to people we support both in service and via external agencies.
  • Lead on and provide training and support for others to refer to appropriate external agencies when any health concerns or deterioration occurs, lead on any health emergencies that occur while in the service.
  • Advise and update specialist and non-specialist professionals on the health care needs of the people we support developing written reports to support information, effectively communicate service user individuals needs in meetings as required.
  • Lead and support others to effectively communicate with all people we support, carers, families and other stakeholders, reporting any compliments, concerns, or complaints according to Brainkind policy and effectively managing them as required.
  • Communicate effectively with the interdisciplinary team and external agencies, lead on communication with other services such as a safeguarding, CQC or others as required.
