Description:

Lead the development of the Lincolnshire MHDLDA Alliance Housing Workstream, including the development of a housing strategy Be a key member of the joint accommodation strategy group and play a key role in supporting the activities of the sourcing group. Lead the development and delivery of the strategic objectives for the above. Create and lead forums for housing development initiatives, taking direction and steer from the joint accommodations strategy group and developing a shared programme of work Provide reports to the Mental Health and Dementia Joint Delivery Group on the workstream delivery, progress and key risks. Lead the strategic engagement with external partners to develop the required partnerships needed to deliver greater integration of mental health and housing services and strategies.
For example identification of housing need as well as developing a set of trusted landlords Lead the Trusts work on optimising patient discharge solutions as they relate to housing by addressing key housing issues, including: Identifying existing barriers to discharge across the Trust. Educating/upskilling colleagues in Duty to Refer and homelessness reduction act obligations of Local Authorities. To try to prevent LPFT ever needing to discharge people as NFA. Identifying and working with partners to design solutions that are fit for peoples needs.
Develop a range of long-term settled and short-term transitional accommodation options that enable discharge at the most appropriate time, into the most appropriate accommodation depending on an individuals care/support/housing needs and vulnerabilities. Be the Lead Professional for Housing and any future housing support team, ensuring that professional standards are defined and met, continuing professional development is available, and additional supervision is available for more complex areas of practice. Represent housing issues at relevant trust-wide and system wide forums. Promote the importance and impact of housing more widely/externally to all partners across Lincolnshire.
Reduce dependency on Residential and Nursing Care To raise awareness of the impact of housing on our patients and the impact it has on NHS and Local Authority services (e.g. reduced demand on services by keeping somebody well at home for longer). Build relationships and partnerships with housing providers whose values align with those of the MHDLDA Alliance to develop new innovative housing options. Ensure that Lincolnshire is at the forefront of good practice development, at local, regional and national levels.
Create opportunities for research and evaluation activity within the housing workstream, working with colleagues in the trust and external partners
