Description:
We are looking to recruit highly motivated, innovative and enthusiastic Health & Wellbeing promoter to join our Healthcare Service at Feltham HMPPS/YOI
Feltham HMPPS/YOI looks after children from 15-18yo and adults from 18 and over. By joining our team, you will have the opportunity to be involved in contributing to development of the service that will have a real impact on promoting health and well-being for this vulnerable population and impact on their future life orientation.
We believe that the best health care is delivered by multi-disciplinary teams working well together and in partnership with other teams and services to provide seamless care. Health & Wellbeing promoters will work as part of a team and contribute to the success of the team by being an effective role model and support with health campaigns like smoking cessation, sleep hygiene, nutrition/dieting, etc.
The Health & Wellbeing promoter role is responsible for the delivery of compassionate assessments, care and health promotion services, providing a healthy programme with stakeholders. Nurses work closely with the young people and adults who use our services, their families, friends, and carers and play an important contribution in how they experience our services. The post holder will join a team of nurses, from a variety of backgrounds in terms of experience and training, to care for the patients holistically.
They will provide a comprehensive health promotion service and work as part of an integrated healthcare team which includes primary care, mental health and substance misuse. Health & Wellbeing promoters are expected to be kind and responsive but professional and informative and contribute to the quality of the services we provide by:
- ensuring the best possible clinical outcomes by using up-to-date skills and adhering to evidence-based policies and procedures
- ensuring the patients using our services have a good experience by respecting, empowering and working in partnership with people throughout the care planning and discharge process.

We have a Health and Justice Services Directorate, within which sits the ‘Children and Young People’ Service line working in partnership with ‘Youth Custody Service’ colleagues to provide responsive, trauma-informed primary care, mental health, neuro-developmental and substance misuse services to the young people and young adults within our care.

To be a confident, autonomous practitioner who takes responsibility for the care they give to patients but also take responsibility for supporting the work of others in the team
To plan, deliver and evaluate interventions for treatment as clinically appropriate using evidence-based models. Co-ordinating multi-disciplinary clinical practice to provide focused and integrated care and treatment programmes/packages.
To ensure processes/protocols are in place and followed for effective assessment, planning, implementation and evaluation of care for those children with physical, mental health and substance misuse problems.
To safeguard vulnerable patients in line with Trust and Oasis Restore safeguarding children policy, and support other team members with safeguarding concerns
