We are excited to offer opportunities for a band 2 member of staff to join the Discharge Hub team at North Tees and Hartlepool NHS Foundation Trust.
The successful candidate will be expected to work as part of a multidisciplinary team, assisting with the delivery of therapy and rehabilitation to patients who are referred to therapy services.
To provide support to the multi professional team by undertaking appropriate duties following assessment and planning of requirements by a registered nurse. Additionally to provide support by undertaking non nursing and ancillary duties.

As this is a Trustwide post, the successful candidate may be required to work between the Stockton and Hartlepool localities.
You will be expected to work flexibly and if required be available for both extended day and seven day working. In return we will provide you with robust induction support and the opportunity to develop with the guidance of experienced Senior staff. We actively support CPD. We pride ourselves on the friendliness of our service.
