Description:
We have a fantastic opportunity to join our existing district nursing team.
This post is primarily working in the Out of Hours District nursing team that is based at the Connected Palliative Care HUB. The role is mainly night shifts but includes rotation onto day shifts within District Nursing and Palliative Care.
At night, the team with the support of the Palliative Urgent Response Nurses provide urgent care and support to district nursing and palliative patients in crisis with the aim of maintaining care at home wherever possible. You must be willing to work night and day shifts in the community, have a UK driving license, access to a vehicle to use at work.
