Bupa Telehealth design and deliver a range of telephonic and digital health programs and services for Bupa customers. Supporting Bupa’s purpose of helping people live longer, healthier, happier lives, our team of Health Coaches use their clinical expertise and intricate knowledge of the health system to deliver support and guidance to our customers through multiple channels. Our programs are designed to help our customers feel supported, informed and understood as an active participant in decisions around their health and wellbeing.
We are currently recruiting Health Coaches with current Nursing or Dietitian experience to join our growing team of health professionals.
Responsibilities:
RESPONSIBILITIES
- Assist customers in developing action plans to address opportunities for health improvement that are valuable to the customer
- Assess health status and provide person centered coaching and support to help customers self-manage their condition/risk factors and achieve their health goals
- Connect customers to programs or services that are right for them via outbound and inbound channels
- Guide customers to build on their knowledge, skills and confidence to support self-management of their health goals
- Manage individual caseload of customers scheduling and managing all coaching interventions in line with program guidelines
- Achieve individual productivity and call quality measures including new customer enrolment targets, caseload management and program completion targets
- Opportunities to participate in various continuous improvement projects to enhance customer experience
To be successful in this role you will require:
- Health Care professional qualification can include, Nursing or Dietetics
- Current unrestricted registration with AHPRA (or appropriate registration body) required.
- Extensive experience working within diverse clinical acute and/or community health setting where sound clinical judgment essential
- Experience in Health Coaching is highly desired
- Strong communication skills with a demonstrated ability to educate and motivate individual
