QUALIFICATIONS
- Possess a high school diploma, GED, or equivalent education
- Have under one year of relevant professional experience
Responsibilities:
WHAT YOU’LL DO
- You will review reports to find patients needing adult preventative, diabetes, and other clinical services. You’ll use patient records to create reports for care coordination.
- Coordinating care is a team effort. Work with various teams and resources. Help find community resources for those in need.
- Customer service is essential. You interact with patients, recommend services, schedule appointments, and assist with paperwork. Resolving concerns for patients and families is also part of your role.
- To help patients manage health, provide approved education and telephonic engagement to ensure adherence to care plans.
