Description:

As an integral part of the virtual ward, the post holder will be responsible for undertaking a range of delegated clinical care duties in the community setting for patients who are being cared for by the virtual ward. This will include carrying out daily visits to patients within their own homes who have ongoing acute care needs, to undertake health monitoring including the accurate recording and monitoring of clinical observations. The post holder will also support the remote monitoring of patients using agreed digital technology, undertake clear and accurate documentation in the health records, deliver health education and act as a patient advocate where required, signposting to other services to facilitate support of their ongoing healthcare needs.
Providing clinical observations under the direction of the registered nursing staff on shift.
Prioritise and plan own workload based on the needs of patients.
Competently undertake clinical tests for the purpose of assessment and diagnosis which include blood glucose monitoring, venepuncture, bladder scanning, ECG, blood pressure, oxygen saturation and temperature checks.
Liaising with other professionals and agencies that can support the patient whilst on the virtual ward and as part of their discharge planning.
Acts on own initiative when delivering patient care.
Conducting remote monitoring of patient vital signs using a dashboard and notify any registered nursing staff if the patient is outside their baseline.
Be prepared and able to drive to patient visits who may live anywhere within the geographical area covered by the virtual ward.
Participate in induction programmes for new staff and training programmes for students.
