Description:

We require a friendly, motivated, hardworking committed band 2 health care assistant to join our Dermatology team. You need to demonstrate excellent communication and organisational skills; you are required to be self-motivated with initiative. You will work independently within the multidisciplinary team always ensuring the patients needs are met. You will be working under supervision of a registered health professional.

Duties will mainly be supporting and delivering the smooth running of the clinics, Preparation of the clinical area, preparation of and organization of patients notes.

Mid Cheshire Hospitals NHS Foundation Trust (MCHFT) provides good quality, safe and effective healthcare to the people of Cheshire and beyond. The Trust, which manages Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford, was established as an NHS Trust in April 1991 and became a Foundation Trust in April 2008.

SERVICE DELIVERY

  1. Participate in the assessment, planning delivery and evaluation of patient care, following agreed plans, policies, procedures, and clinical guidelines, independently carrying out tasks and activities relevant for the job role/place of work, which have been agreed and assessed as competent in practice
  2. Work collaboratively within the multidisciplinary team/agencies to ensure that patient
    needs are met
  3. Involve patients, relatives/carers in the delivery of care, providing reassurance and
    support to assist their understanding and co-operation
  4. Prepare the clinical area for safe practice by ensuring that cleanliness is maintained,
    materials and equipment are ready for use, including clearing away afterwards
  5. Provide escort and transfer for patients as directed by the multidisciplinary team
  6. Recognise and respond appropriately to urgent and emergency situations
  7. Provide information to enable the effectiveness of care to be evaluated and records
    maintained
  8. Contribute to the recording and storing of information, ensuring it is securely held,
    accurate, complete and legible and can be retrieved promptly when required.

Responsibilities:

A proficient support worker who actively participates in the assessment, planning,
implementation and evaluation of care by working collaboratively with the multidisciplinary team/others to meet the needs of patients and their families. An agreed range of duties are
carried out independently following assessment of competence. The support worker also
assists with the learning and development of others in the workplace.
