Description:

JOB OVERVIEW:

Under the direction of the leadership team, the Health Equity Navigator will build effective working relationships with affiliated and non-affiliated physicians, senior centers, hospital providers, family members/care givers and community organizations to improve individualized patient care coordination and reduce barriers across the health care continuum. They will coordinate physician office visits, outpatient treatment, surgical and specialist referral appointments, transportation arrangement, facilitate access for patients to offsite physician offices and/or hospital departments to provide appropriate support. The navigator will also support the implementation of new workflows while supporting the expansion of the availability of comprehensive health services by integrating resources to optimize care delivery.

QUALIFICATIONS:

Required:

  • Bachelor degree is required
  • Experience in project management in the healthcare field is required

Preferred:

  • Strong medical terminology background is preferred.

Responsibilities:

ESSENTIAL FUNCTIONS:

  • Coordinate appointments with RWJUH outpatient departments, BHMG providers and private physicians to ensure timely delivery of diagnostic, treatment services and specialist referrals
  • Facilitate financial support and helping with paperwork
  • Coordinate and at times accompany patient to physician offices and outpatient departments and provide language translation or interpretation services
  • Arrange transportation for eligible patient as required
  • Organize patient schedules and manage their appointments to ensure they receive services in a timely manner
  • Inform and educate patient on financial aspects of care and linking patient to financial resources as needed
  • Facilitate communication between the patient, family members, and healthcare providers to ensure the highest patient satisfaction and quality of care
  • Acts as community outreach coordinator for the Chinese community and work closely with Public Relations Department to create educational activities, events, workshops and special projects for the Chinese Community.
  • Frequently meets with physicians and other health care providers around the region to keep them up-to-date on new faculty, services and equipment available at affiliate hospitals
  • Provides a two-way communication link between BHCS and referring physicians by managing patient s follow up visits, outpatient testing and ensuring continuation of care
  • Serves as a personal contact for physicians or their practice managers in order to address problems, issues or concerns that need immediate attention
  • Educate physician office staff regarding hospital referral process
  • The information shared with the Health Equity Medical Program Coordinator is highly valued and is used by the affiliate hospitals for planning and evaluating future services and programs
