Responsibilities:

  1. To support the integrated 0-19 Healthy Child Service locality team in providing an innovative and high-quality service in an environment that enhances client care and promotes child and family wellbeing.
  2. To support the ongoing assessment and review of clients
  3. To work under direct supervision in supporting the delivery of Universal and targeted interventions to children, young people and their families across the 0-19 HCS. This may be in a variety of settings including the family home and at times dealing with emotional and distressing circumstances.
  4. To support in a Venue or Group setting with the delivery of Child Health reviews, 6 week, 1 year and 2 year assessments and Pathway to parenting sessions (P2P).
  5. To complete administrative tasks and organization of appointment bookings.
  6. Participate in continuous service improvement within the organization/service priorities.
  7. Maintain effective and appropriate verbal, written and electronic communication.
  8. Assist in the delivery and planning of programmes/activities either with individual parents/carers or groups of parents/carers which aim to improve outcomes for children and young people which may involve some manual handling.
  9. Work in collaboration with the locality skill mix team and partner agencies e.g., ECFS, Family Hubs, and Early Help to provide a safe and effective service to children and their families.
  10. To be aware at all times of the needs of client groups and to promote and encourage positive health choices.
  11. To plan and manage own day to day tasks which may include organisation of health education events.
  12. Duties will include working under supervision in a variety of settings.
  13. Weighing and measuring babies and children in line with the health growth protoco
