Description:

Overview:
The Heart Failure nurse collaborate with the Heart Failure team in all aspects of patient care, with a focus on coordinating care needs of the heart failure population from the time of referral through end-of-life. Assists in the clinical management and resource allocation for patient with HF. Support the growth and expansion of the Heart Failure program by actively participating in performance improvement initiatives to enhance care, processes, and outcomes. Coordinates and collaborates with other Heart Failure nurses in the market. Manages CardioMEMS and other remote monitoring. Uses critical thinking to triage patient phone calls and collaborate with medical providers to titrate medications within scope of practice. Provides evidence-based disease self-management education and serves as a resource for our heart failure patients. Accurately and timely documents in the medical record.

Qualifications:

  • Education: 4 Year / Bachelors Degree BSN, preferred.
  • Experience: 3 years of experience in Telemetry, CCU, Cardiac Stepdown; Minimum of 3 years of Clinical RN experience, required.
  • Licensure/Certification: Registered as RN in NC or compact state, required. Certification as a CHFN, required.

  • Demonstrates interdisciplinary leadership skills, superior interpersonal communication skills, and able to communicate patient’s needs to providers.
  • Organization skills, critical thinking skills, and clinical experience/expertise in HF management
  • Ability to work independently on their own as well as collaborative with the interdisciplinary team.
