The Community Stroke Therapy Team provides specialist, patient-centred community and outpatient-based rehabilitation for adults with stroke. You will be joining an enthusiastic, MDT, working alongside colleagues in specialist stroke services, the stroke and neurology inpatient team and the Neurological Rehabilitation Team.
We are looking for a n experienced, dynamic, highly motivated Physiotherapist , with excellent clinical and leadership skills in the management of neurological patients, specifically Stroke.
As the Team Leader, you will jointly lead a team of Physiotherapists and Occupational Therapists. Alongside a band 7 Occupational Therapist, you will oversee the delivery of a high-quality therapy service, to ensure patients achieve optimal rehabilitation . You will have a clinical caseload.
We are looking for someone with substantial post - graduate experience, including MDT working in neurological and community settings . You will have been involved in service development projects and have experience of managing and supervising staff.
This is a community role requiring you to travel between sites.
To provide leadership and clinical expertise to the Community Stroke Therapy team. To provide day to day line management to a multidisciplinary team of staff in an outpatient and community setting.
To be responsible for planning and co-ordinating caseloads and staffing, making adjustments on a daily basis as required to ensure effective running of the team.
To assess, develop and implement evidence based therapy treatment utilising specialist theoretical knowledge and relevant practical experience within the field of stroke rehabilitation.
To undertake lead responsibility of the supervision, training and appraisal of all grades of staff within the team including therapy students.
Working without direct supervision using own initiative to interpret operational policies and procedures.
To undertake all aspects of clinical duties as an autonomous practitioner, working within the ethical framework provided by the rules of professional conduct.
To liaise with the wider MDT and multi-agencies to facilitate best patient care.
To perform advanced therapeutic assessment of patients with diverse presentations, to provide a diagnosis and treatment plan.
To be the main person initiating and co-ordinating changes to working practice and service delivery.
