Home Care Aide

UPMC Home Health is hiring a full-time Home Care Aide to work Monday-Friday 8:00 am-4:30 pm. No weekends or Holidays. This position offers excellent benefits including mileage reimbursement, tuition reimbursement, and growth opportunities
Provide designated care under the direction of the primary service provider in the home to achieve identified goals. Function as an extension of skilled nursing services and rehabilitation services in the house.

• Must maintain annual competency evaluation scores as determined by regulatory requirements
• Attends compliance training and adheres to the organization’s standards of conduct, policies, and procedures.
• Proven ability to facilitate change.
• Focuses on customer service and continually strives to perform their job duties in a manner that will result in optimal patient satisfaction.
• Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
• Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization.
• Participates in all required agency meetings.
• Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patients’ condition to appropriate professional staff.
• Maintains and completes documentation accurately and promptly while in the patient’s home and within the agency and regulatory standards.
• Provides safe and direct personal care to patients in the home setting under the direct supervision of appropriate professionals.
• High School Diploma or GED - Required
• One (1) year experience - Preferred
• Certified Nurse Aide Preferred
• Minimum of 75 hours of Nurse Aide training
• Required. If no previous nurse aide training, the applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire.Licensure, Certifications, and Clearances:
• Automotive Insurance
• Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
• Driver’s License
