Responsibilities:
ABOUT THE ROLE AND WHERE YOU WILL BE WORKING:
The Home Care Service Coordinator provides excellence in applying a wellness and reablement approach and coordinate services for clients to achieve their goals and improve their health and wellbeing outcomes. To work collaboratively with clients, internal and external stakeholders to assess, plan and facilitate the delivery of care to clients considering their assessed care needs strengths and goals.
This position is located at our Bran Nue Dae centre, co-located with Germanus Kent House in Broome (2,044.5 km north of Perth)
TO BE CONSIDERED FOR THIS ROLE, YOU’LL NEED:
- Experience leading a small team
- Experience working with Aged Care or similar industry, particularly managing client outcome-based program funding e.g. CHSP, HCP etc.
- Experience working within Indigenous communities/sector
- Ability to work with internal/external stakeholders to achieve program and client goals
