Description:

UPMC Healthplan has an exciting opportunity for a Home Modification Case Manager position in the Community HealthChoices Department. This is a full-time position working Monday - Friday daylight hours with flexibility. This is a remote position. This candidate must reside in the state of Pennsylvania to be considered for this role. The primary function of this role is to collaborate with internal and external parties to facilitate home modifications for participants across the state of Pennsylvania to make their homes safer and more accessible with the goal of keeping them in their home / community setting.

Responsibilities:

  • Planning and coordinating home modification related projects, including evaluating initial requests, reviewing occupational therapist’s evaluations, approving project scopes, and overseeing the entire process from start to finish.
  • Communicating with therapists, contractors, suppliers, service coordinators, and other internal stakeholders as needed throughout the project to ensure that everyone is aligned. Managing project budgets and timelines, ensuring that projects are completed within budget and on time.
  • Preparing reports and documentation for each project, including cost estimates, progress reports, and close out reports.
  • Maintain and protect PHI (Protected Health Information).
  • Identify and troubleshoot issues with participants cases to ensure safety and satisfaction.
  • Participate in training programs as available/requested.
  • Bachelor’s degree preferred or 2 years of equivalent relevant experience may be considered in lieu of degree.
  • Minimum of two years of general business experience required.
  • Competence in MS Office Suite, Adobe, and general PC Skills required.
  • Experience in project management, home renovations, cost estimation, or general construction is plus.
  • Knowledge of health insurance, Medicaid, and Medicare products is a plus.
  • Organizational, interpersonal, and communication skills required.Licensure, Certifications, and Clearances:
